For many volunteer positions in Middle School Ministry, including being a driver or adult leader for one of our events, adult volunteers need to submit a completed background check permission form at least one week prior to volunteering. Background checks are valid for 3 years. Background checks are valid for 3 years. Volunteer Covenant Form Volunteer Information Form
